Our Team


Louis E. Costello, M.D. is board certified in Child, Adolescent, and Adult Psychiatry and has been in practice in the DFW, North Texas area since 1996.  He has been practicing in his current location in Highland Village, Texas since 2012.   Dr. Costello trained at University of Texas Southwestern in Psychiatry, and he is a Fellow in the American Psychiatric Association.  He is recognized for his continued dedication to excellence in clinical care and makes every effort to use the most up-to-date psychiatric assessment, medication management and treatment strategies which are conservative, compassionate, and research-based.

Dr. Costello was the first TMS clinical provider in the state of Texas and in the DFW area to use FDA approved TMS treatment for Major Depressive Disorder.  Because of Dr. Costello’s mission to provide the highest quality medical care to his patients, he endeavors to create a professional and calming environment at DFW TMS resulting in the most comprehensive and individualized psychiatric care which includes the following:  evaluations, referrals to other medical and psychological caregivers, individual therapy and parent and family guidance. Dr. Costello’s goal is for each patient to become as reasonably joyful and functional as their individual strengths will allow them to be.

Meredith Hake, RN, FPMHNP completed her Master of Science in Nursing in 2007, and is a Board Certified Family Psychiatric Mental Health Nurse Practitioner. She received an Honors Bachelor of Science in Nursing degree from the University of Texas at Arlington, and a Bachelor of Arts in Psychology and Spanish from the University of Texas at Austin. She has spent the last 10 years working extensively with patients of all ages with depression, anxiety, autism, PTSD, bipolar disorder, ADHD, and other mental health disorders. Prior to completing her Master’s degree she worked for 8 years with children and their families at Cook Children’s Hospital in Fort Worth, TX.

Meredith has worked with Dr. Costello at DFW TMS since 2010. She is a certified rTMS technician, and has attended Neurostar Pioneer’s Conferences in Chicago and Atlanta. Meredith has seen the benefit of TMS therapy in her patients, and truly believes that TMS is a key part of treatment for medication resistant depression.

Outside of work, Meredith enjoys spending time with her husband and two children. She is actively involved with her children’s school as well as her church and her community. She is also involved with the Nursing Program at the University of Texas at Arlington, working to help train future Psychiatric Nurse Practitioners.

Donald Van Hooff, RN, FPMHNP is a Board Certified Family Practice Mental Health Nurse Practitioner, graduating from nurse practitioner training from the University of Texas at Arlington in 2005.   He began working with DFW TMS in 2009.  He has over 30 years nursing experience, working with children, adolescents, and adults, and has been providing comprehensive mental health services through collaborative care with Dr. Costello since 2005.  In addition to the years of experience as a Psychiatric Nurse Practitioner providing care for patients with a wide range of issues, Don is dedicated to helping individuals with difficult to treat or medication resistant MDD to become as completely well as possible.

Don holds a TMS Certificate from the Association for Convulsive Therapy’s course on Transcranial Magnetic Stimulation.  He has attended TMS conferences in San Francisco, Chicago, and Atlanta, and is certified from Neuronetics, the makers of the first FDA approved TMS treatment system.

Don’s other interests include volunteering in third-world countries, playing chess, and watching foreign films.  He views his decision to become a mental health nurse practitioner as one of the best he has ever made.  His thoughtful dedication to his profession and his patients helps drive Don to help each individual under his care.

Suzanne, TMS Coordinator and Certified TMS Technician

Suzanne has served as a Certified TMS Technician and as the DFW TMS Coordinator since 2014. With a Marketing degree from Texas State University and a 25 year career in sales and marketing with four Fortune 100 Corporations, Suzanne brings a different perspective to DFW TMS.  As the TMS Coordinator she works every day to help educate patients regarding how TMS treatment works.  If patients are using insurance, she then ensures the patient’s insurer is aligned with prescribed treatment plan. Once patients are approved, Suzanne is an integral part of the healing process for our patients as she is in the treatment room with them facilitating the CBT programming we use with each DFW TMS patient.  Her contagious enthusiasm, love for life and care for our patients brings an added value to our TMS services! Suzanne loves her job, because she is able to physically watch patients become free of depression in just 36 quick treatment sessions!

In her spare time Suzanne enjoys reading and organic vegetable gardening.  She is a bee-keeper and also maintains several composting earth worm farms. She is very involved in her church and active in our community. Suzanne loves to travel and has attend the International Clinical TMS Society meetings in Atlanta and San Diego.

Cynthia, Office Manager and Certified TMS Technician

Cynthia is The Practice Administrator for DFW TMS.  She recently moved to the DFW area from her home state of Mississippi where she was the Office Manager for Kindred Home Health and Hospice, formerly Gentiva Hospice, for the past 5 years.  Cynthia graduated with a degree in Accounting from Mississippi State University and brings to DFW TMS over 30 years of medical office experience including working in the business departments of several medical clinics including The Surgery Clinic of Tupelo.  Cynthia has worked for two hospice companies, North Mississippi Residency Center, and also served as a K-5 Health Educator for 10 years with The Community Health Department of North Mississippi Medical Center.  She is an amazing addition to our team here at DFW TMS, as she just makes everything better!

Cynthia enjoys being of service to patients of all ages and is also passionate about being an advocate for several organizations including The Alzheimer’s Association, The Diabetes Foundation, The Susan G. Komen Race for the Cure, and The Humane Society.

Lorna, Medical Assistant and Certified TMS Technician

Lorna joined DFW TMS in spring of 2017.  She graduated from DeVry University with a Bachelor’s degree in Telecommunications Management. Lorna worked in finance and accounting in the telecom industry for over 17 years. After leaving the corporate world, she decided to start a new career in the medical field, and now has nine years of service in the Medical community.

Lorna is a Certified TMS Technician, Certified Medical Office Manager (CMOM), Certified Clinical Medical Assistant (CCMA) and a trained Allergy Technician (AT).  At DFW TMS Lorna serves as both a TMS Technician and as our Medical Assistant.  She helps create a smooth flow of medication information between our clinicians and our patients or their pharmacy.  Additionally due to her diverse background, Lorna provides energy and input to our team wherever needed.  She loves to find solutions to complex situations and therefore she is a true asset to our team.

Kaitlyn, Front Office Assistant and Certified TMS Technician

Kaitlyn is an intern here at DFW TMS.  She is currently completing her Bachelor’s Degree in Exercise Sports Science from Texas State University in San Marcos, TX.  Kaitlyn has worked in an array of fields including customer service and marketing and she hopes one day to pursue a career in medical sales.  In her spare time Kaitlyn enjoys exercising in a variety of different settings and spending time with her family and friends.

As an invaluable asset to our clinic, in addition to being trained as a Certified TMS Technician, Kaitlyn checks our patients in and out and assists in other departments as needed.  Kaitlyn is excited to be a part of the team here as DFW TMS as it has been an eye-opening experience for her to witness the benefits of psychiatric care and the impact of TMS treatments on patients who are dealing with Major Depressive Disorder.
